LOA Requests:
 

If an LOA is required for an approved authorization, please send an encrypted email to providerservices@chcnetwork.org with the authorization number.

Health Center Credentialing:
  All Medical providers working in CHCN health centers must be credentialed by Alameda Alliance for Health (AAH) including volunteers, on-call, and speciality providers.

To get started on credentialing a  provider at the clinic , please email AAH at Deptcredentialing@alamedaalliance.org using the information in the template below:

1. Provider Name
2. Provider NPI
3. CAQH#
4. Supervising physician name + attach Supervising Physician Forms (if provider is a mid-level)

Once a provider has completed credentialing with AAH, please submit the following forms as an encrypted email to providerservices@chcnetwork.org .

1. New Clinic Provider Request Form
2. NPO attestation Form
